What Is the Best Treatment for Sagging Upper Arms? Arm Lift vs. Liposuction & Nonsurgical Alternatives

An arm lift (brachioplasty) is often considered the most effective treatment for sagging upper arms, especially when loose or excess skin is the primary concern. Removing and tightening skin typically creates a toned, defined contour that other treatments can’t always achieve.

However, not all upper arm concerns are the same. In some cases, stubborn fat or a combination of fat and skin laxity play a significant role, making liposuction or nonsurgical options more appropriate.

As a board-certified plastic surgeon specializing in body contouring and lipedema treatment, my goal is to help you understand the underlying cause and guide you toward a treatment plan that aligns with your anatomy, goals, and lifestyle.

What Causes Sagging Upper Arms?

Sagging upper arms are most often caused by weight loss, aging, genetics, or reduced skin elasticity. When the skin has stretched and cannot contract on its own, the upper arms may develop a loose or “bat wing” appearance that does not improve with exercise.

When it comes to choosing the best sagging arm skin treatment, my recommendation typically depends on what is actually creating the concern. Some patients need skin removal, some require fat reduction, and others may benefit from skin tightening or a combined approach.

Is an Arm Lift the Best Treatment for Sagging Upper Arms?

Yes, an arm lift is typically the best treatment for moderate to significant loose upper arm skin. The procedure removes redundant skin to create a firmer, smoother arm contour.

An arm lift may be the right choice if you have:

  • Hanging or crepey upper arm skin
  • Skin laxity after weight loss
  • A “bat wing” appearance when lifting your arms
  • Discomfort from rubbing, heaviness, or excess tissue
  • Limited improvement from exercise or nonsurgical treatments

During surgery, I place the incision along the inner arm, usually between the armpit and elbow, so that excess tissue can be removed, and the upper arm can be reshaped. Recovery involves some discomfort and tenderness for a few weeks post-op, but most patients describe it as manageable.

For the right candidate, this approach offers a more dramatic and lasting improvement than nonsurgical skin tightening alone.

Arm Lift vs. Arm Lipo: Which Is Better?

Choosing between an arm lift and arm liposuction comes down to skin quality. Liposuction removes fat, while an arm lift removes loose skin.

Liposuction may be a better fit if you have:

  • Localized fat in the upper arms
  • Good skin elasticity
  • Mild skin laxity
  • A desire for smaller, more contoured arms without skin removal

At Hagopian Plastic Surgery, I use advanced VASER® and SAFELipo® techniques to remove stubborn fat with greater precision and make recovery as smooth and comfortable as possible.

An arm lift may be more suitable if you have:

  • Loose or hanging skin
  • Moderate to significant laxity
  • Skin that is unlikely to contract after fat removal
  • Changes after major weight loss or aging

It’s essential to point out that liposuction does not tighten loose skin. If skin laxity is already significant, removing fat alone may make sagging more noticeable. For some patients, the best result comes from combining liposuction with an arm lift to reduce excess fat and tighten the overall arm shape.

Can Nonsurgical Skin Tightening Help Sagging Arms?

Nonsurgical or minimally invasive skin tightening can help mild to moderate arm laxity, but it cannot replace an arm lift when there is significant excess skin.

At Hagopian Plastic Surgery, less invasive options may include:

  • QuantumRF: Uses radiofrequency energy to tighten the skin and stimulate collagen production with little to no downtime. It is designed to address skin laxity when excess fullness is not the primary concern.
  • BodyTite®: Uses radiofrequency energy to reduce fat while tightening the surrounding skin. It is often combined with liposuction when both excess fullness and loose skin are concerns.
  • Morpheus8: Combines microneedling with radiofrequency energy to improve skin firmness, texture, and collagen remodeling. This option provides skin tightening without incisions but typically requires multiple treatments to achieve optimal results.

While QuantumRF and BodyTite both use radiofrequency energy, they address different needs. QuantumRF generally provides more intensive skin tightening but is not designed to target fat cells. BodyTite offers skin tightening and fat reduction, making it an ideal option for patients with loose skin and excess fullness in the upper arms.

These treatments may be a good fit if you are not ready for surgery, have early laxity, or want subtle refinement. During your consultation, I will explain whether skin tightening is likely to meet your goals or whether surgery will provide a more meaningful result.

What Should I Know About Arm Lift Scars?

An arm lift requires an incision to remove excess skin, typically placed along the inner arm, where it is less visible in many natural arm positions. The trade-off is a firmer, more contoured arm shape in exchange for a scar that fades over time but does not disappear completely.

My goal is to place the incision thoughtfully, remove the right amount of tissue, and support your healing with enhanced recovery after surgery (ERAS) protocols and scar reduction therapies. Most arm lift patients find that the improvement in arm shape, comfort, and confidence makes the scar worthwhile.

How Do I Choose the Right Sagging Arm Skin Treatment?

The right treatment for sagging upper arms depends on your skin elasticity, how much excess fat you have, the laxity of your skin, your medical history, and your aesthetic goals. Some patients need an arm lift, while others are better candidates for liposuction or RF skin tightening. Still, others benefit from a combined approach. This may include pairing an arm lift with procedures such as an upper body lift, thigh lift, lipedema surgery, or a drainless tummy tuck to achieve a beautifully balanced silhouette.

As your guide and partner in your transformation, I ensure you have a thorough understanding of your options, empowering you to make informed decisions about your treatment plan.
